We can support your independent living skills and help you to manage your life and choices with confidence.

We provide Outreach and Floating Support Worker services to help you manage your daily life and assist you to get out and about to appointments and to get things done around the house.

If you want support workers to come and visit you in your home and you live in Islington or Camden speak to your Care Manager and they will contact us on your behalf.

If you have a Personal Budget or have your own funds we can provide trained and friendly Personal Support Workers across north London through our Finding and Matching Service.

Supported Housing Read more >>

Centre 404 runs a number of supported housing projects to people in Islington.

Our support workers are highly trained and experienced in delivering person centred approaches. We are an accredited and approved provider by Islington Council and we retain an ‘Excellent’ rating with the Quality Care Commission.

If you are looking for a place to live in Islington we may have supported housing available. Contact your care manager for an assessment and complete a Centre 404 application form. Your application will be given to the Islington Housing Panel and a Broker to make a decision.

If you are receiving Direct Payments or a Personal Budget and you want to move out we can help you find a place of your own. Our support workers can assist you to fill in an application form to find flats in your local area. They will help you through the whole process until it is time to move out. They can co-ordinate your move and support you with your bills.

Supporting High and Complex Needs Read more >>

Centre 404 is an experienced provider of supported housing for people with high and complex needs. We ensure that our staff are highly trained and have the experience and understanding to support people with profound and multiple disabilities and additional health needs. We work closely with the individual, family carers and professionals to develop a support plan that meets the needs and wishes of the people we support.

If you are looking for supported housing, you will need to contact your Islington Care Manager for an assessment. You will also need to fill in a Centre 404 application form. Then your application will be given to the Islington Housing Panel and broker for a decision.

If you have questions or concerns about supported housing for people with high and complex needs we can also provide advice, information and advocacy services to family carers as well as many support groups and activities.

We offer many clubs and activities, trips and fun experiences for children, young people and adults.

Our friendly, experienced team can provide support for groups of people to access a variety of social and leisure activities at our Centre and out in the local area.

If you have a personal budget we can help set up activities for you and support you to get out and about and see things that are going on in your local area.

You can also choose activities you like to do and we will help you to set up new groups. If we know there are a group of people interested in doing the same things we can set this up as a new group.

You can join our Friendly Association that has been set up and is run by a group of people who receive support from Centre 404s independent living schemes and housing projects. They meet regularly to talk about issues affecting people with learning disabilities. The Friendly Association is for anyone who is a member of Centre 404.

Find a support worker

Finding and Matching Service Read more >>

If you have a Personal Budget you can choose your own Personal Support Worker with our Finding and Matching Service.

We can offer a wide range of person centred services and a team of highly trained and experienced Personal Support Workers.

We employ and manage staff on your behalf. This includes all recruitment, training, payroll and HR.

All Support Workers have CRB clearance and at least two references before they start working.

We can also manage your Personal Budget for you as an Individual Service Fund (ISF).
